TRANSFER OF RECORDS & XRAYS
In advance of your first visit, please make every effort to work with your prior providers office to complete record release paperwork allowing them to send your patient records and/or current radiographs to our office. If they are unable to send them electronically to [email protected], please bring hardcopies with you.
